The following are the steps to take when a client has a referral from a physician to receive an eye examination or has already seen an ophthalmologist and requires eyeglasses.

1. Complete a certified referral form in its entirety. This form may be found on the forms page of the Health Department's Ryan White website.
    
2. After the form has been filled out completely, signed and dated, make the appointment for your client. Do Not Fax the referral form to the eye clinic or your contract coordinator until the appointment has been made. When making the appointment please identify that the appointment is for a Ryan White client to ensure appropriate billing stream.
