Community Alternative Program /Children CAP/C

Case ManagerThe CAP/C Program provides services for medically fragile children under 21 who are at risk of institutional care. By providing in home nursing care, case management, and other supports, CAP/C can help these children stay at home with their families.

  • Skilled nursing
  • Respite care
  • Therapy services
  • In-home aide services
  • Durable medical equipment
  • Medical supplies
  • Disposable incontinent products for children age 3 and older
  • Home mobility aides (wheelchair, ramps, doorway widening, safety rails, non skid surfaces, hand held showers and grab bars)
  • Pediatric nurse aide care
  • Home modifications
  • Vehicle modifications

CAP/C parent handbook will answer your questions about the CAP/C program and provide you with information and resources to help you become an informed and active participant in your child’s care.

 

Client Eligibility

Children must be under the age of 21 years. Clients must also qualify for CAP/C Medicaid at Department of Social Services.

Cost and Payment Information

There can be a cost to clients for services provided.
