Community Health Assessment - We want to Hear from You

Mecklenburg Healthy CaroliniansEvery 4 years, Mecklenburg County Healthy Carolinians and the Mecklenburg County Health Department conduct a community health assessment to identify, describe and prioritize issues affecting the public health of our community. Action plans are developed to focus on the prioritized health areas.

For the 2010 assessment, we would like to invite your participation as a county resident in this process by asking you to fill out a short survey. The entire process should take less than 10 minutes. Responses are completely anonymous and results will be reported only by group.

Because we are interested in hearing from people across the county, we would also ask you to forward the survey link to your friends, colleagues and co-workers. We apologize in advance for any duplicate messages. If your organization has clients who might be interested in participating, please invite them as well.

The survey is available in Spanish as well as English. We also have hard copies in English and Spanish and would be willing to deliver and pickup if you have clients or groups who might prefer to participate on paper. We really would like to hear from as many people as possible and we very much appreciate your help to make this happen.

 

Thank you so much for providing us with your input for this assessment. Results will be presented during a priority setting activity at the end of October and will also be used to recommend new and enhance current strategies for promoting health.
