Company Operating Certificate information…
The following checklists, application forms, and enclosures will help guide prospective and current for-hire transportation company owners through the process of applying for or renewing their company operating certificate.
Company operating certificates must be renewed annually. All taxicab company operating certificates expire as of July 31st and must be renewed during the month of July, each year. All other passenger vehicle for hire company operating certificates expire as of August 31stand must be renewed during the month of August, each year.

Driver/Chauffeur Permit information:
The following checklists, application forms, and enclosures will help guide prospective and current drivers and chauffeurs through the process of applying for or renewing their driver or chauffeur's permits.
Driver and chauffeur permits must be renewed annually. These permits expire on your birth date and must be renewed during the same month.

DRIVER PERMIT RENEWAL & TRANSFER/ADD COMPANY FORMS
DRIVER PERMIT REINSTATEMENT (Any permit expired 30 days up to one year will be treated as a new permit requiring an initial application process.)
DRIVER PERMIT REINSTATEMENT (Any permit expired one year or more will be treated as a new permit requiring that you re-take the Driver Training Class, pass the class, and then complete the initial application process.)
