On January 3rd
the state Department of Health and Human Services (DHHS) issued a statement regarding the miss-mailed Medicaid cards. The department said the mistake was “human error in computer programming and the quality of the assurance process in printing the new Medicaid identification cards”.
Please take the card you received incorrectly and immediately shred or cut it into very small pieces. You can also turn the card into your local county Department of Social Services office:
Mecklenburg County Social Services

Please continue to use your old NC Health Choice card until you receive your new card.
If your child’s card was incorrectly mailed, you will receive a letter from DHHS stating that you were affected by this breach and included will be a statement of the Medicaid services rendered using your child’s Medicaid ID Number.
