Mecklenburg County is a grantee recipient of Part A Ryan White Treatment Modernization Act federal grant funds.  The Ryan White Program provides HIV-related health and support services. This website was created with the goal of improving communication and services throughout our Transitional Grant Area (TGA).

Important Update: Dental Referrals & Client Re-Evaluation Form
First Dental of Huntersville has opened a new office location at Belvedere Family Dentistry - 2734 Rozzelles Ferry Rd. Please see updated dental referral procedure for details.
 
Please see the Client Re-Evaluation Form - This form is to be completed semi-annually or in the event of an unanticipated event(s) or changes in client status. This form is used as a guide to assist case managers in properly documenting and updating client’s file for reassessment.

Updated Tools
The Case Management, Fiscal Monitoring and General Monitoring Tools have been updated. The only changes to these 3 tools are that monitoring standards which were added in FY 2011-2012 are now highlighted in “RED” for quick reference. You must fully with these standards in FY 2012-2013. Please download these new versions and discontinue using previous version.
